If you (and/or your children) are new to our studios, we require any and all participants/legal guardians to completely fill out the standard Move and Motion release before any activity is to be performed.
You can receive and complete the form at Move and Motion studios – or for your convenience, you can do so right now by downloading the PDF document below and filling it out properly.
Be sure to give it to your child’s instructor or coach before ANY activity, class, private lesson, etc., is to begin.
This is the only way that participation is allowed by ANYONE, this includes birthday parties, dance, yoga, any practice, performance and ALL activities that we have at Move and Motion.
